What PPO Blue is

Highmark PPO Blue is a program where you contract with a network of preferred doctors.

PPO stands for preferred provider organization. Highmark contracts with a network of “preferred” doctors, hospitals, and facilities that agree to discounted rates. As a PPO Blue member, you can use those in-network providers for the lowest cost, or you can go out of network and still have the plan pay a share. Compare that to an HMO, which generally pays nothing for out-of-network care outside of emergencies, and the value of a PPO for treatment becomes obvious: it preserves your choice of provider.

The four numbers that decide what you pay

Deductible

The amount you pay before the plan starts sharing costs. PPO plans typically carry two deductibles, one in-network and a higher one out-of-network, and they usually accrue separately.

Coinsurance

After the deductible, you and the plan split costs by percentage. In-network splits are more generous; out-of-network splits ask more of you but still cover a meaningful share.

Out-of-pocket maximum

The ceiling on what you pay in a plan year. Once you hit it, the plan pays 100 percent of covered services. For a residential stay, this number often matters more than any other, because a multi-week admission can reach the maximum, capping your total cost.

Allowed amount

Using PPO Blue out of network

Out-of-network benefits are the feature most members have never used and do not realize they own. Using them for treatment looks like this:

Using PPO Blue out of state
