This Notice of Privacy Practices describes your rights and our duties under federal law. Protected Health Information (PHI) is information about you, including demographic information, that may identify you and that relates to your past, present, or future physical or mental health or condition; the provision of healthcare services to you; or the past, present, or future payment for the provision of healthcare services.

We are required by law to maintain the privacy of your PHI, provide you with notice of our legal duties and privacy practices regarding your PHI, notify you following a breach of unsecured PHI related to you, and abide by the terms of this Notice of Privacy Practices.

Confidentiality of Addiction Treatment Records
The confidentiality of alcohol and drug abuse patient records maintained by Bright Paths Recovery is protected by federal law and regulations.
Generally, we may not disclose information identifying you as a patient or relating to your substance use treatment unless you provide written consent, disclosure is permitted by a court order, or disclosure is made to medical personnel in a medical emergency or to qualified personnel for research, audit, or program evaluation.
Violation of these federal laws and regulations is a crime and may be reported to appropriate authorities.
Federal law does not protect information about crimes committed on the premises or against staff, threats to commit such crimes, or suspected child abuse or neglect, which must be reported under state law.
Relevant laws include 42 U.S.C. §§ 290dd-2, 290ee-3 and 42 CFR Part 2.
Uses and Disclosures
Your PHI may be used or disclosed as permitted, required, or authorized by law.
Among Bright Paths Recovery personnel, PHI may be shared between staff members who need the information to provide diagnosis, treatment, referral, payment, and healthcare operations. This includes clinicians, nurses, administrative staff, and billing personnel.
We may disclose PHI to the U.S. Department of Health and Human Services to determine compliance with HIPAA regulations.
PHI may be disclosed to business associates who perform services on our behalf. All business associates are required to protect your PHI, use it only as permitted, and comply with HIPAA and 42 CFR Part 2.

You may request a list of certain disclosures made within the past six years. Requests must be made in writing.
You may request limits on how your PHI is used or disclosed. We are not required to agree except where legally mandated.
If you pay in full for a service, you may request that information about that service not be shared with your health plan.
